Forklift Pre-Shift Inspection

29 CFR 1910.178(q)(7) · examined before each shift, at least daily

Fill it in at the truck, print it, file it. Nothing is sent to a server — the form keeps a draft in this browser only. Items that do not apply to your truck's power type can simply be left blank.

OSHA's wording is that a truck shall not be placed in service if the examination shows a condition adversely affecting safety. That judgement is the operator's and the employer's. This page records what you found; it does not decide whether a truck is safe to run.

Tick what you checked and found sound. Anything not sound goes in the findings table below rather than being ticked.

Function test — engine running

Findings

Anything not sound. Marking a finding as taking the truck out of service is a decision with consequences — the truck should not be placed in service until it is corrected.

One shift is fine on paper. But an OSHA visit asks for the daily records and for each operator's three-year performance re-evaluation under 1910.178(l)(4)(iii) — and almost nobody can produce the second one.
