Vaccine Fridge Temperature Log

CDC Vaccine Storage and Handling Toolkit · twice daily, min/max · VFC provider agreement

Print it, put it on the unit, and record the start and end of each workday. Nothing is sent to a server — the form keeps a draft in this browser only.

If a reading is out of range, the CDC's process is to label the affected vaccine DO NOT USE, store it properly but separately, and contact the manufacturer or your state health department to determine whether it is still viable. This page never makes that determination and states no stability limit for any product. It records what happened and what you were told.

Daily readings

Current, plus the minimum and maximum since the last reading. The min/max is the point: a unit that swung overnight and recovered looks perfect on a spot check.

Date AM current AM min AM max AM init. PM current PM min PM max PM init.

Temperature excursions

Anything outside the range you recorded above. Note who you called and what they told you — that answer, not this form, decides whether the vaccine can be used.

A month fits on a page. Three years does not

The VFC provider agreement expects these records to be retained for years and produced at a site visit, and a lapsed data-logger calibration certificate invalidates the readings it took. Paper handles neither well.
